Troupe Director & Chaperone Responsibility

The following forms are needed to complete your Troupe’s Registration. Please help our volunteer board by taking the time to complete forms accurately and fully. Incomplete forms could put your troupe’s registration at risk. All forms must be submitted by January 31.

• Troupe Director Responsibility – Will be emailed to Troupe Directors after registration closes.
• Chaperone Responsibility – Will be emailed to Troupe Directors after registration closes.
