The following forms are needed to complete your Troupe’s Registration. Please help the our volunteer board by taking the time to complete forms accurately and fully. Incomplete forms could put your troupe’s registration at risk. Please also ensure your student’s send in their Supplemental Forms in early. There is no grace period for these submissions. All registration and supplemental event forms must be postmarked by January 31.
